Service Description
By booking this service, you are reserving one flash fine-line tattoo with an independent third-party tattoo artist. Tattoo designs must be selected from the artist’s provided flash sheet. Custom designs or modifications are not available. This booking is valid for one tattoo only. All tattoo services are performed by an independent vendor, and availability is limited. Participation is voluntary. Participation in tattoo services is entirely voluntary. By choosing to receive a tattoo, attendees acknowledge that tattooing involves inherent risks including, but not limited to, skin irritation, allergic reactions, infection, or dissatisfaction with the final result. The Creepy Retreat and its organizers are not responsible for the tattoo services provided, the outcomes of tattoo procedures, or any aftercare or medical concerns arising from participation. All tattoo services are provided by an independent third-party vendor. Attendees are responsible for reviewing aftercare instructions and confirming that they are medically able to receive a tattoo. A government-issued ID may be required by the artist. No refunds or credits will be issued by event organizers for tattoo services.
Cancellation Policy

You must complete this form once booked: https://form.jotform.com/253215230318042 I have been fully informed of the inherent risks associated with the process of getting a tattoo and the importance of proper aftercare and healing of a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to scarring, infection, difficulties in detecting melanoma, allergic reactions to tattoo pigment, latex gloves, and/or soap. I also state that I have not excluded any important health conditions that I have had or may have. I have been informed of the potential risks associated with getting a tattoo, yet I wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks that may arise from tattooing. Therefore, I hereby release any and all persons representing The Creepy Book Club, LLC, Zabreena Sauceda (@emotionally.tatted)/ Bailey Cromwell (@bloved.ink)/ Kayla Shields (@finelinesbykayla) from all responsibility. I accept any and all responsibility myself for the consequences that might stem from my decision to have any tattoo done by one of these artists. I agree to not sue The Creepy Book Club, LLC, Zabreena Sauceda (@emotionally.tatted)/ Bailey Cromwell (@bloved.ink)/ Kayla Shields (@finelinesbykayla) for damages from my decision to have any tattoo work. I agree for myself, heirs and legal representatives to hold, The Creepy Book Club, LLC, Zabreena Sauceda (@emotionally.tatted)/ Bailey Cromwell (@bloved.ink)/ Kayla Shields (@finelinesbykayla) harmless of all damages, actions, causes of action, claim judgements, costs of litigation, attorney's fees and all other expenses which might arise.
